Office Hours & Appointments

Our office is open from 8:00 AM to 5:00 PM (we are closed between noon and 1:00 PM), Monday through Friday. Appointments may be made by calling (813) 870-2528.

You will be seen as soon as possible, but please understand scheduled patients are seen first.

If you are having a medical emergency please dial 911.

If you have questions or concerns before or after office hours, please dial (813) 931-2800. Our answering service will take your call and notify one of the doctors. Your call will be returned promptly.

New Patients

If you are a new patient, please click here to download and print your new patient forms. Completion of these forms is required in order to better serve you as a new patient.

Reminder to All Patients

• Co-pays are due at time of service, expect a five day turnaround for referrals if NOT a Direct Admit.
• Bring a list of medications with you to your next doctor's appointment.
• Remind front desk of any address, insurance, phone or employment changes.
• A patient survey is available for completion at the front desk.
• For all prescription refills you must give the office 24 hour advance notice.
