Thank you for your visit. We have created this area just for you to conduct research on various medical trends as well as learn what we’ve been up to here at South Tampa Medical Group. New patients can download important medical forms here and fill them out before your visit.

What to Expect on Your First Visit to Our Office

Your first visit to a new office can often be filled with uncertainty and many questions. We go out of our way to make sure your experience with us as comfortable as possible and that your questions are answered. As a new patient, we ask that you download and print the new patient forms located at the bottom of this page. Completion of these forms will help reduce your wait time when you arrive for your appointment.

An initial visit with us may consist of required laboratory work such as bloodwork or EKG, followed by a doctor's examination and consultation.

As a reminder to all patients, we ask that you make note of the following requests to ensure a comfortable office visit:

• Be sure to bring your original insurance card. We can not accept photo copies.
• If you are changing doctors or are coming in on referral, please notify your insurance company of the change prior to your visit.
• Co-pays are due at time of service, expect a five day turnaround for referrals if NOT a Direct Admit.
• Bring a list of medications you are currently taking.
• Remind the front desk of any address, insurance, phone or employment changes.
• A patient survey is available for completion at the front desk.
• For all prescription refills, please give the office 24 hour advance notice.
